Informed consent form for parents and guardians
Thank you for registering your child(ren) for a Canada Learning Code experience!
Canada Learning Code is committed to reaching people traditionally excluded from tech and code, and increasing diversity in the tech and code field. We will invite your child(ren) to complete surveys to help us learn more about them and their learning experiences.
What information will we collect about your child(ren)?
The 10-minute surveys will ask about how the Canada Learning Code experience has impacted their interest, confidence and skills in tech and code. We will not collect their names in the surveys.
Do your child(ren) have to participate?
No, your child(ren)’s participation in the surveys is voluntary. They do not have to participate if they do not want to. Even if they agree to participate, they can still leave at any time. They do not need to answer certain survey questions if they do not want to. Your and your child(ren)’s decision to participate in the surveys or to answer all survey questions will not affect your child(ren)’s participation in any Canada Learning Code experiences in any way.
This information will also be provided to your child(ren) before they complete the surveys.
What are the possible benefits and risks for your child(ren)?
There are no direct benefits for your child(ren) if they take the surveys. However, the information they provide will help us understand how our experiences have reached and impacted young participants like your child(ren), and how we could improve our experiences.
We do not believe that your child(ren) will experience any significant risks to their wellbeing by taking the surveys.
How will we protect your child(ren)’s information?
Your child(ren)’s responses to the surveys will be linked to their identifying information you entered at registration so that we could track our learners’ long-term learning outcomes. Any information about your child(ren) is confidential. We will securely store it and only use it for program evaluation and improvement purposes. We will not share it with anyone outside of Canada Learning Code. Results and reports will only contain aggregate information about all participants of Canada Learning Code experiences, without any of your child(ren)’s identifying information.
